***New Requirement: Immunization and Physical Records***
The Massachusetts Department of Public Health (DPH) has recently updated the 105 CMR 430 Minimum Standards for Recreational Camps for Children. As a result, we are now required to collect immunization records for all summer program participants.
This is a new process for both our department and the DPH, and we appreciate your patience as we implement these changes.
What You Need to Do
Submit Immunization and Physical Records:
- Deadline: Records must be received at least 1 week prior to your program’s start date.
- Late Submissions: Records received after this deadline will not be accepted, and your child will not be allowed to participate.
- Refund Policy: Unfortunately, no refunds can be given for missed deadlines due to contractual obligations with our vendors.
How to Submit:
- Email the immunization record to: Anthony Tolson Jr, Recreation Supervisor
atolson@framinghamma.gov
- Email Subject Line: IMMUNIZATION REPORT – [Child’s Full Name]
- Email Body: Please include your child’s name again and list the dates and names of the program(s) they are enrolled in. Please send 1 email per participant.
